New Student Registration

NEW STUDENT REGISTRATION

Please contact the Counseling Assistant, Teresa Erdkamp to schedule an appointment at 402-786-2765 x 208.  New student appointments for the 2017-2018 school year are by appointment only.  Registration throughout the school year will be by appointment only.

There are many forms that must be completed in order to register; all required forms will need to be completed prior to coming to your registration appointment.  All of the forms needed for registration are available for you to download and complete on-line. You may not be able to save the results (with the form completed).  You can access the district website at:

www.district145.org

Click on Parent tab, click Parent Forms. If you do not have Internet access, please contact the Counseling Office and the forms will be mailed to you.  You will find all of the forms that you need to complete here.

Required High School Student Forms

High School Records Release Form (New Students Only) - Complete this form if you have not already done so. Please drop this form off at the Counseling Office or scan the completed form and e-mail to teresa.erdkamp@district145.org.  This form grants us permission to receive your records from your previous school.  Please note, though, that if YOU GET THESE RECORDS DIRECTLY FROM YOUR PREVIOUS SCHOOL PRIOR TO YOUR APPOINTMENT we will be able to fully register you.  

IF YOU DO NOT HAVE RECORDS with you at your appointment, we will not be able to fully register you or complete your schedule which may delay your enrollment in school.

We must receive current and official transcripts, immunizations, Special Education records (if applicable) and a birth certificate in order to complete the process.   It is in your best interest to contact your school and either have these records sent to us directly or bring them with you to your appointment.

School District 145 New Student Registration Form – This record is used to enter demographic information.  It is critical that contact information be accurate.

WHS Handbook/Restrictions Form (Required - All Students) Students and Parents must indicate on this form that they have viewed the Handbook via the Internet or that you prefer a paper copy of the Handbook and understand its contents.  Your student will receive a Waverly High School planner after school starts.

Dental Health Form - This is required by the state of Nebraska for 10th Grade only.  Parent & Dental Signatures are required.

If you are moving from out-of-state to Nebraska, a physical is required.  The physical needs to be dated after May 1, 2017.

Affidavit of Residence (may be required) - If you are in the process of building a home in School District #145, but are not currently living in the home, you will need to complete this form.  This form must be notarized.  

Temporary Delegation of Parental Powers (may be required) - Complete this form with a notary only if you are a guardian or are currently seeking guardianship.  If you currently have guardianship of your student, please provide documentation of such at your appointment for placement in the student’s cumulative file.  All guardians must be current residents of School District #145.

2017-2018 Free & Reduced Lunch Application - This form can be turned in during your appointment.  Current lunch prices for the current school year are not available until August.

Other Information that may be needed

IEP & MDT documents - If your student receives Special Education services, please get current IEP and MDT documents from your previous school.  It would be beneficial for you to bring these documents to your appointment.

High School Club & Sport Forms – You will need to complete the following Activity forms in order to participate in a school activity.  A Preparticipation Physical Form and Physical will need to be completed prior to the fall athletic practices.

Compliance Form - Required for all students who are out for a sport or want to belong to a club

Preparticipation Physical Form  - This includes the Physical Evaluation Form and the Physical Evaluation History Form (Required for all sports grades 7-12)

NSAA Student & Parent Consent Form (Required for all sports grades 7-12 and if in a WHS Club, Student Manager, or Trainer grades 9-12)

Activity Trip and Interscholastic Activities Transportation Form (Required if student is driving off-campus for a sport or club activity.)  This form is located in the Activity Guidelines Handbook.

A 2017-2018 Course Book can be found on-line, click Schools, then click the High School, then click on Counseling Office for you to begin reviewing course selections.  Your course choices will be dependent on your current grade, graduation requirements, and college choices.  Scheduling will be reviewed and discussed at your appointment.
