Health Requirements

7th Grade and new students (in any grade) MUST turn in evidence of a physical done by an MD, DO, APRN, NP or PA and dated 6 months prior to the first day of school. This also includes up-to-date immunizations including Tdap. Vision and Hearing Screens are also required and can be done as part of the physical unless otherwise done by a Specialist and must be dated within 6 months prior to the start of the school year.

Dental screenings are required as well and must be completed within 6 months prior to the first day of school.

If no evidence of Dental, Vision or Hearing exams is provided, these students will be screened during school screenings.

Sports Physicals

ALL students planning on participating in any athletic activity MUST turn in a Sports Physical dated AFTER MAY 1. Your student's Well Child Physical will be accepted IF there is a statement written or the provider has signed they are cleared for activity. If your child saw their Healthcare Provider prior to May 1, they will need to be seen again for the Sports Physical. Students cannot participate in any activities without turning in proof of a physical exam without clearance for activities.

Illnesses

Your student must stay home if they for at least 24 hours without the use of medication for the following reasons:

  • Have a temp of 100.0 or higher

  • Diarrhea

  • Vomiting

Students must be in attendance at school for at least half of the day in order to be eligible to participate in or attend any activities that same day.

Hi!

I live in Waverly with my husband, 4 boys, and our dog, Flo. When I'm not at school, you can usually find me attending my boy's activities, making cakes, and being outdoors. Summer is my favorite!

I've worked in healthcare in various settings since 2008, including the ER, NICU, Women's and Adolescent Health, and Family Practice & Urgent Care. I am thrilled to be a resource of health and support for students and their wellbeing. I not only bring many years of experience, but I also work with a fantastic team of staff here at WMS to ensure your student has the best Middle School experience!
