New Instructions and Forms
Juniors Seniors are allowed college visits and job shadowing days as a part of their high school experience and as preparation for their futures beyond the high school walls. We encourage each student to make use of these valuable opportunities. The student and/or family are responsible for contacting the college or shadow site and providing transportation.
Procedures for Job Shadowing or College Visiting allow a student to be absent without question as long as the visits/shadows do not affect a student's overall attendance requirements. Some college visit requests or job shadow requests may not be supported if that request violates the eight-day attendance policy or if a student's absences are excessive. Other school activities will be taken into consideration in all attendance appeal situation. Please contact Kristen Delehant, Assistant Principal, to discuss any situations in which attendance is a concern.

COLLEGE VISIT VERIFICATION FORM
Student MAY pick up the College Visit Verification Form from the MAIN OFFICE or print off from the high school website prior to the visit.
*The form is also attached below.
NOTE: This form MAY BE REQUESTED if attendance is a concern. Though the form in not required, it may help a student if he or she is in a "loss of credit" situation due to attendance.
THE COLLEGE VISIT EXPERIENCE WILL SHOW FOR ATTENDANCE AS A PARENT REQUEST. If the form is requested by the attendance office or if you choose to turn it in, the form should be given to Brooke in the main office.

ENGLISH 11 JOB SHADOW EXPERIENCE AND REQUIREMENTS:
As part of a junior English Project, juniors may take a day for a shadow experience. The form that is to be turned into the English 11 teacher is attached below.
Students may NOT directly job shadow a parent/guardian for the English 11 course or to be considered for attendance requirements.
DISTRICT STATEMENT: Parents/Guardians agree to release, hold harmless, indemnify and defend School District 145 (hereinafter the District) from any claims, loss, or liability arising out of or related to any curricular activity including any that may occur before, during, or after the attendance of my child/ward at curricular activities, whether due, in whole or in part, to the negligence of the District or any other theory of liability against it, or any other cause. Parents/Guardians understand that there are inherent and other risks involved in travel to and from, and attendance at curricular activities. Parents/Guardians freely and voluntarily accept these risks for my child/ward and all other risks associated with travel to, and attendance at curricular activities. Parents/Guardians also understand and agree that the District is not responsible for the time or manner in which my child/ward may arrive at or leave curricular activities.
